testing: More restructuring for the Nose source

* Split the general-purpose testing discussion into the testing/
directory, keeping only nose stuff in testing/nose.
* Pull example discussions into example-specific READMEs.
* Condense testing/nose/instructor.md, and add links to upstream docs.
