Cleaned up TOC/LOF/LOT and bump to version 0.13

This fixes a line break bug with latex reported by Daniel Cross, wher
figure and table entries had looked like

  1.1
      A table float .  .  .  .  .  3

rather than the expected

  1.1 A table float .  .  .  .  .  3

The bug did not occur with pdflatex.
